Patient Forms

Please print and complete the New Patient Information Packet before your appointment at Bindal Neurosurgical Clinic.  
Remember to bring:

  • Insurance Card

  • Driver’s License

  • MRI or CT (The actual films and the paper report)

  • List of your current medications

  • Referring Doctor’s address, phone, fax number

  • Referral if required by your insurance

  • Medical records

  • Cash or check (no credit cards accepted)
